A checklist will be emailed to each Client prior to our consultation of items that will be required to complete the Business Plan,  for Program Management Consulting, and Project Management Consulting.

Checklist for Mortgage Assistance:
1. Provide a copy of the delinquent letter from your mortgage company.
2. Provide a copy of the foreclosure letter if applicable.
3. Proof of whether you’re employed or unemployed by providing two most recent paycheck stubs, a most recent unemployment letter from the Georgia Department of Labor, TANF, SSI, SSDI, Worker's Compensation, or a copy of your 1099-Form if you are self-employed. 4. A copy of your driver’s license and social security card.
 

Checklist for Rental Assistance:
1. Provide a copy of the delinquent letter from your landlord.
2. Provide a copy of the eviction letter if applicable.
3. Provide a copy of your legal signed rental agreement.
4. Proof of whether you’re employed or unemployed by providing two most recent paycheck stubs, a most recent unemployment letter from the Georgia Department of Labor, TANF, SSI, SSDI, Worker's Compensation, or a copy of your 1099-Form if you are self-employed.
5. A copy of your driver’s license and social security card.
 

Checklist for Utility Assistance:
1. Provide a copy of the delinquent utility bill.
2. Proof of residence by providing a copy of your driver’s license.
3. Proof of employment or unemployment by providing either two most recent paycheck stubs, unemployment letter from the Georgia Department of Labor, TANF, SSI, SSDI, Worker's Compensation, or a copy of your 1099-Form if you are self-employed.
